Three high school girls are bored and decide to conduct a seance with a bunshinsaba - a traditional Korean Ouija board. As the spirit is called by Eun-jung (Lee Yoon-ji), her sister, Eun-seo (Jeon Hye-bin), scolds the girls. Eun-jung jokingly wishes that the ghost had taken Eun-seo away and that if they do not send the ghost away, it will kill them. Eun-seo while at her sink in her kitchen, notices that the sink is clogged, when she pulls at the filter, she pulls out a large clump of human hair. The sink turns on by itself and floods the floor, which causes Eun-seo to vomit large amounts of water, where a ghostly spirit emerges from and proceeds to kill her. The amnesiac Min Ji-won (Kim Ha-neul), who is an expert swimmer, begins to have nightmares of a ghost from her forgotten past, which seems to be connected to water. She is confronted by an estranged friend, Yu-jung, who tells her that because of Ji-won, "Su-in" is coming for them. Yu-jung is soon found dead. Ji-won slowly remembers that she used to lead an elitist clique back in high school which consisted of herself, Eun-seo, Yu-jung, and Mi-kyung. Before she became part of the clique, Ji-won was friends with Su-in (Nam Sang-mi), but her ego changed her and she ended up regarding the introverted Su-in as a laughingstock. Ji-won visits her last remaining clique member, Mi-kyung, who is confined to a mental hospital, but the latter lashes out at her. Ji-won learns from Su-in's mother that the event that led to her amnesia was a trip to a forest with the girls, from where Su-in had gone missing and was never heard from again. After she leaves, Mi-kyung is drowned by the ghost. Heading to a waterfall in the aforementioned forest with Jun-ho, Ji-won finally remembers what had happened a year ago: after arriving at the bottom of a waterfall, Ji-won asked Su-in if she can swim, then pushes her into the spring. Su-in comes up and the girls laugh finding the prank hilarious. However, the other three girls then pushed Ji-won in as a joke, but upon seeing that she hadn't resurfaced they started to panic. Ji-won eventually resurfaces but reveals that she can't swim, Su-in dived in to try to save her. During this, Mi-kyung had an asthma attack, but managed to reach her inhaler with Yu-Jung's assistance. When Su-in tried to save her, Ji-won unfortunately struggled a bit too much and accidentally ended up causing her to end up stuck to the bottom of the river. Ji-won after resurfacing went back to try and save Su-in, unfortunately her foot was stuck between some underwater rocks and despite Ji-won's attempts, she unfortunately drowned. Mi-Kyung and Yu-Jung stare in shock as only Ji-won resurfaced, while Eun-seo sadistically smiled back at her. Ji-won after remembering what had happened, dives into the river and finds Su-in's corpse still in pristine condition and hugs her as an apology. Jun-ho dives in after her and brings her back to the surface; where Ji-won explains what she remembered and how guilty she felt about causing that to happen. Together they alert the authorities, who manage to bring Su-in's body back to the surface, where Su-in's grieving mother identifies her. Jun-ho asks a detective how Su-in's body could be in such pristine condition after a whole year had passed, and he reveals it was due to all the arsenic in the water. It had preserved her body for an entire year as arsenic even repels fish, which explains why her body never decayed even after staying underwater for an entire year. Jun-ho also asks about what will happen to Ji-won, the detective reveals that since it was just a horrible accident she most likely won't be charged. Ji-won returns to her mother and reveals what had transpired, only for her mother to reveal herself as the ghost of the real Ji-won as she had been possessing her the entire time. She forces Su-in to realize the horrifying truth: Ji-won and Su-in had swapped bodies during their struggle to get out of the spring. "Ji-won" did not lose her memories because she was never Ji-won in the first place; she is actually Su-in. Ji-won's spirit has been terrorizing her and the clique for revenge and to gain her body back, and now she inhabits her mother. Slowly, Ji-won realizes that she is in fact Su-in and remembers how she had desired to take Ji-won's place which she had somehow managed to accomplish in the spring. Her mother vomits a large volume of water and collapses. Ji-won crawls out of the puddle of water but Su-in cuts her wrist so Ji-won can't take her body back. She collapses unconscious from the blood loss and later reawakens in a hospital, where Ji-won's boyfriend, Jun-ho, reveals that he had come to visit her at her house and managed to save her and mother. Jun-ho assures Ji-won/Su-in that he will always stay by her side and hugs her, unaware that she is actually Su-in. After recovering, Su-in is later seen walking through the fish market where her mother, who believes her daughter has died, works.
