In 2004, college student Zachary began filming a sociology project for class, which involved capturing footage of a full lunar eclipse. Alongside him are his childhood friend Jacob and Jacob's younger sister Elise, who is days away from starting college herself. The group leaves Morgantown, West Virginia, and travels to the small town of Sutton for a weekend camping trip aimed at documenting the eclipse. They film locals about superstitions surrounding the moon, encountering both skeptics and a deranged man who warns that the eclipse marks an impending disaster. As they venture deeper into the woods near Flatwoods, technology malfunctions and strange footage begins to appear. A hidden trap door is discovered in a forest clearing. Though reluctant at first, they descend into an underground bunker where time freezes at 10:27:04 and strange writings and images suggest a history of cryptid experimentation. The facility appears to be tied to a secret government operation known as "Project Butterfly," involving legends such as the Flatwoods Monster, the Grafton Monster, Hell Hounds, and the elusive, puppet-like cult leader Indrid Cold. Separated inside the bunker, Jacob discovers a house frozen in time and a ghostly woman who is violently pulled into a vent by a monstrous arm. Zachary and Elise navigate a puppet-filled supply closet, finding disturbing drawings and fragmented footage of rituals involving Appalachian creatures. After Jacob escapes, he radios for help, only to be answered by Indrid Cold himself. When Jacob is attacked by the Veggie Man and killed, Elise and Zachary try to escape the underground labyrinth, but not before Zachary sacrifices himself to save Elise and possibly a missing classmate named Edith. Elise emerges alone into the woods, still clutching the camera, its timestamp never advancing. She flees through the forest and reaches the town of Sutton, where she pleads for help at the local police station. In a final scene, a hand-belonging to Indrid Cold-moves figurines across a map of West Virginia, knocking over the human pieces and placing the Flatwoods Monster at the center, revealing that everything the group experienced was part of a larger plan. The screen cuts to black.
